Veterans who have learned a homelessness to give the opportunity to get the health care in the community, a study of VA researchers found. The majority of veterans surveyed had positive views of community care that they received. However, the coordination challenges exist when veterans receive in the Community as well as in the Community and VA in health care.
"Nights without accommodation, current homelessness and chronic homelessness did not seem to limit the access of veterans to the community care," concludes the researchers. "The results imply that the Evolution of VHA has hindered no health access for very vulnerable veterans from a direct provider to a payer of health services."
The results appeared on April 6, 2021, the issue of the journal medical care .
In 2014, the veteran choice program enabled that legitimate veterans in the Community of private health care in the Community have received health care. The program uses a network of providers to expand the care of VA patients, which are far from VA systems that have lived far away from VA facilities or long waiting times for appointments. The program was extended by Mission Act 2018.
In support of veterans, care outside of VA will gain your access to health services, it can also lead to coordination issues. Some worry is that veterans, which result from more than one source, can lead to fragmented care and poor clinical results.
This may be true, especially in endangered populations such as homeless veterans. Homeless veterans often have complex health needs with high, chronic medical, psychic health and substance usage conditions. They often have difficulty to access services. Since VA focuses on the care coordination in order to manage the often complex medical needs of homeless veterans, there is a concern that the provision of motion outside the central VA system can cause problems.
To investigate how the veteran choice program has influenced this population, researchers examined nearly 5,000 veterans in an e-mail survey in 2018. The survey was conducted before the Mission Act was implemented in 2019. The survey introduced veterans with recent experiences of homelessness of 26 VA medical centers. Fourteen percent of respondents were currently homeless, 15% had protection in the last six months, and 19% experienced chronic homelessness.

Older veterans, female veterans, and those who were managed in a homeless patient-oriented care team (H-PACT) were more satisfied with the community care. The results also showed that veterans with psychological distress and financial hardness were more likely to use the community care, a unique find for this study.While most respondent veterans were satisfied with their care, the care coordination suffer from something that the researchers were expecting. Veterans who used both VA Health Care and Community Care reported the poorer nursing coordination as those who used only VA. Satisfaction with the Community Care was also lower for veterans with fewer personal resources (d. H. Traveling, Financial Eponishment and Limited Social Support).
According to study first author. Audrey L. Jones of the Salt Lake City Health Care Systems, a place to search for teaching lessons to improve the nursing coordination for homeless veterans is in the H-pack system. H-PACT is a coordination effort, which is specifically geared to the needs of veterans, experiencing homelessness. "We found that homeless experienced veterans were managed in the H-pact in H-pact compared to other pacts, were more satisfied with their time to get the community care," explains Jones. "They also had cheaper perceptions of VA Care coordination. If you are closer to help in H-PACT in H-pact, especially improving care coordination for susceptible veterans who are managed in traditional VA primary care environments."
The results are part of a larger study by VA Health Service Research and Development (HSR & D) to determine which factors of H-Pact are the most geared. The researchers, the Dr. med. Stefan Kertesz of the Birmingham VA Medical Center and the University of Alabama, Birmingham, have hoped to identify specific components of H-PACT - as on-site services, personnel and team culture - which promote positive care experience for homeless veterans.
